Most of BASF referrals come from healthcare professionals or from social care. We welcome referrals at any stage in the stroke pathway and from people months, or even years, after their stroke.
Everyone referred is contacted and offered a home visit from a Stroke Care Co-ordinator. The Stroke Care Co-ordinator will assess each person giving them ample opportunity to talk through the issues that are affecting them. The Co-ordinator is very experienced in life after stroke care and will signpost to relevant services according to need. With the permission of the client, they will also complete a screening test for anxiety and depression. Research shows the strong link between anxiety and depression and poorer outcomes and our work focuses particularly on emotional and psychological wellbeing.
Depending on the needs of clients, Co-ordinators will offer a range of services including further individual home visits, one-to-one counselling, carer support and a range of groups. Some of these are time-limited groups whilst some offer long-term social opportunities.
